How To Use The AmeriPlan Health ® Program

How Does AmeriPlan Health ® Work? Below is an explanation about how to use each part starting with the Primary Care Physician.

1. Primary Care Physicians
|

When you join AmeriPlan ® you will need to pick a Primary Care Physician to be your family's doctor. You don't need to fill out any paperwork or forms to make them your doctor; you just call AmeriPlan ® when you're ready to go see them the first time. This is the procedure to follow the first time.

Locate A Provider - Find a PCP in your area, to locate one, follow the
Locate A Provider link at the top of this website. You will notice that the phone number listed next to EVERY doctor is the same, and that number leads to AmeriPlan ® customer service.
- Call AmeriPlan ® - The phone number is 1-866-977-2477; let them know that you are an AmeriPlan Health ® member and you would like to go see your new PCP.
AmeriPlan ® will then call your PCP to ensure they are taking new patients and fill them in on all of the AmeriPlan Health ® pricing and payment procedures. Now it's your turn to call your new PCP.
- Make Your Appointment - AmeriPlan ® customer service will give you their phone number after they speak to your new PCP. Now that everyone is on the same page, go to your appointment and meet your new doctor! Make sure you bring your AmeriPlan Health ® membership card with you so the doctor's office can make copies for their records. At the conclusion of your appointment, pay the doctor the discounted fee for the services they performed.

You only need to follow this procedure the first time you go see your PCP. After that, the doctor's office will keep in their records all of the information about how to bill you.

2. Specialists 3. Ancillary Care
|
|
These two have been combined because the procedure is about the same. Your doctor will let you know when you need these services so you don't need to worry about it until then. When that time comes, your doctor will give you a
Patient Procedure Form that you need to take to the specialist or ancillary care. Then follow the steps listed below.

Locate A Provider - Find a specialist or ancillary care in your area, to locate one,
follow the

- Call The Provider - Let the provider know that you are an AmeriPlan Health ® member and you would like to make an appointment. Make sure you bring your
AmeriPlan Health ® membership card with you.
- Pay The Provider - At the conclusion of your appointment, pay the provider the discounted fee for the services they performed. They will give you some things to bring back to your doctor for analysis.

If you need blood work done in a lab, your doctor will send a fax to
Medstat requesting the service and you can pay the discounted fee for the lab work while you are at your PCP's office. The lab will send the results to your doctor and then your doctor will contact you with the results.

|
In the case of hospital confinement that costs more than $2,500, a patient advocate will be assigned to your case to negotiate with the hospital on your behalf. If you know you need a hospital stay, you can arrange everything before you go. In case of an emergency it will all be taken care of after your hospital stay is over. The procedure to follow is the same for both.

Call
AmeriPlan ® - Their toll free number is 1-866-977-2477. Tell them about your need for a patient advocate and let them know if it's pre or post confinement. They will send you a form (by mail or fax) to fill out with the specifics.
- Fill Out The Form - Send the completed form to the Karis Group in Austin, Texas. The address and fax number for the Karis Group will be on the form.
- A Patient Advocate - will then be assigned to you personally. Your advocate will contact you to help determine your best way to pay. Then your advocate will contact the hospital and make all of the arrangements on your behalf.
